All CSHM students who are interested in participating in athletics are required to register through the homecampus.com website. The link below will connect you to homecampus.com/login, there is also a handout and a YouTube video below that will guide you through a step by step process of the clearance procedures. Students are required to register on homecampus.com/login only once. Students are required yearly to select the sport/s in which they would like to participate, and upload a current physical exam to the homecampus.com website. Preparticipation Physical Evaluation

All CSHM student-athletes are required to have a current physical exam uploaded to the homecampus.com website. A current physical exam must be completed and uploaded every year of athletic participation. Attached below is a preparticipation physical evaluation pdf that you can download and take to your physician (MD or DO) to complete, and upload back into the homecampus.com website.
