The announcement and congratulation form is available for you to submit a lasting memory for your daughter, son, or loved one on the event of their graduation from Cantwell-Sacred Heart of Mary High School in the 2012-2013 yearbook. Just click on the pdf below. It is a wonderful gift to have your thoughts and blessings on this joyous moment in a young person's life celebrated for all time in their CSHM Yearbook.
Just print and fill out the form, include your message and all photographs you would like included in an envelope that you bring to the front office along with your payment. Although you can submit a photo on a cd, it is better that we scan the original photo so that it has the optimal resolution for the yearbook.
The deadline is Monday, March 11, 2013. We cannot accept them at a later date because our deadline to submit the pages is immediately after the announcement deadline.
