Applications for tuition awards from the Catholic Education Foundation (CEF) for the 2021-22 school year are now available and being accepted. For those that are currently receiving a CEF award, remember, that applications must be submitted every year. It is not automatically renewed.
Families with multiple students attending CSHM must turn in an application for each child.
Applications available here in English and Spanish. For those without access to printing, copies are also available in the CSHM office.

What you will need to apply:
Proof of Income*
2019 Income Tax return documents. A copy of the ENTIRE tax packet is needed. *The child must be listed on the tax return* If you receive public assistance such as unemployment, CalWorks/ CalFresh, disability, etc., you must also include a copy of the monthly statement.
*If taxes were not filed, you must complete the NOTARIZED STATEMENT OF INCOME page. Fill in the information and have it signed by a Notary Public.
For incoming and transfer students NEW to CSHM, please ask your current school principal to complete the PRINCIPAL RECOMMENDATION FORM (included). They should fill it out and enclose a copy of your child’s report card from last year. Elementary schools can also email the recommendation form and grades to: firstname.lastname@example.org
*Current CSHM students do not need to do this.

You may bring your completed application along with your tax documents to the CSHM office.
Regular office hours are Mon. – Thurs. 8 am - 3 pm. & Fri. 8 am - 2pm.
